Advance Care Planning for Caregivers
Advance Care Planning (ACP) is a way for you to think, talk and plan for health care that is right for you before a crisis happens. ACP has been shown to reduce the anxiety and distress that can accompany living with serious illness, frailty and end of life for both the person with the illness and the family caregiver. Not only do people with illness report a better quality of living and greater satisfaction with care by engaging in their advance care planning, but families report a piece of mind and a decrease in caregiver stress.
Please join us for this one hour lunch time session as Pat Porterfield RN, MSN encourages you to have a conversation on what Advance Care Planning is, why it is important for you and the loved one you are caring for and the steps involved. Pat was involved in the development of advance care planning within Vancouver Coastal Health and since retirement has continued with public education for advance care planning and co-chairs the BC Centre for Palliative Care Advance Care Planning Public Awareness and Education Working Group.
